Valutazione di un nuovo materiale per il controllo di qualitą della velocitą di eritrosedimentazione

ABSTRACT. In spite of its empirical nature, the measure of the erythrocyte sedimentation rate (ESR) is very frequently used in clinical medecine. The ESR is the "acute phase slow reactant" more often used in medecine. "Fast" markers (i.e. C-reactive protein) are considered not to be alternative but complementary to the "slow" markers: the coupled determination would allow to optimize the achievable information. Among the slow markers alternative to the ESR, the measure of plasma viscosity is proposed, even if poorely used in practice; in analitycal terms it has the advantage to possess quality control materials, while ESR does not. The new material proposed for the quality control of ESR measurement exhibits characteristics suited to the expected scope. The availability of such a quality control material contributes to adjust the analytical reliability of ESR measurement to its widespread clinical use.

BTA-TRAK come indicatore urinario di recidiva da carcinoma a cellule transizionali della vescica

ABSTRACT. We evaluated the diagnostic accuracy of BTA-TRAK, in combination with urinary cytology (UC), in the follow-up of patients with a TCC history. The overall sensitivity of BTA-TRAK, UC and the two combined tests for recurrences was 81%, 87.5%, and 94%, respectively. BTA and UC showed a 100% sensitivity for invasive G3 recurrences. UC sensitivity was slightly higher than that of BTA-TRAK (85% vs. 78%) for superficial lesions, whereas BTA-TRAK (82%) was more sensitive than UC (73%) for G1 recurrences, and by combining the two tests, the sensitivity increased to 93% and 91%, respectively. The differences in urinary BTA-TRAK levels among patients with recurrences, without evidence of disease, with benign/inflammatory disorders and normal subjects were statistically significant (Wilcoxon's test, p<0.05). Thus, BTA-TRAK, combined with UC, is an accurate and highly predictive marker for TCC recurrences, and could replace cystoscopy for patient monitoring.

Valutazione dell'analizzatore COBAS Integra 400 nella determinazione delle droghe d'abuso

ABSTRACT. The COBAS Integra 400 (Roche Diagnostics, Basle, Switzerland) is a random access analyzer for the quantitative determination of enzymes, substrates, specific proteins, therapeutic drugs, drugs of abuse (DAT) and hormones in human specimens. As a part of an international multi-center evaluation for this instrument, we evaluated the analytical performances of DAT applications. Typical intra-assay CVs for Roche positive DAT controls ranged from 0.3-1.8 % (mean CV from 5 runs with 20 replicates) and, for inter-assay, CVs ranged from 0.6-11.0 % (CV over runs with 20 replicates over 5 days). The method comparisons including at least 150 urine specimen for each drug showed a good correlation between the DAT assays on COBAS Integra 400 and our previous routine assays as well as with the Abbott AxSYM. In conclusion the COBAS Integra 400 DAT applications showed good precision and correlated very well with the two compared routine systems.
