La valutazione dello "stato ossidativo" nella pratica clinica

ABSTRACT. Evidence has increased suggesting that a condition of oxidative stress, due to imbalance of oxidative/antioxidant systems, can produce alterations in important biological functions as a consequence of free radicals reactivity towards macromolecules and DNA.Free radical oxidative stress has been implicated in the pathogenesis of a variety of human diseases. Direct measurament of oxidative stress in vivo, especially during antioxiant terapy, is a very complex question, because free radicals are highly reactive, have very short life and are present in very low concentrations.Thus, indirect methods, used for measuring secondary products of oxidative stress, are unspecific and may give conflicting data. Recently a new simple and cheap method to monitor oxidative stress in serum was developed. This test (d-ROMs) detects the derivatives of reactive oxygen metabolites and showed good precision, accuracy, linearity. The "total oxidative staus" include in the biochemical profile also measurements of principal and secondary antioxidant defenses.
Monitoraggio farmacologico della buprenorfina a scopo terapeutico

ABSTRACT. In this work we highlight the utility of a new chromatographic assay of nor-buprenorphine, the main metabolite of buprenorphine, performed with HPLC technique, with the Remedi multi-columns system. We suggest wide intervals of reference values, expressed as nanograms of nor-buprenorphine per milligram of urinary creatinine, well correlated with the assumed dose of buprenorphine (Subutex), as a valid instrument to evaluate the correctness of the given dose and of the compliance of the patients referring to the Drug Addiction Service

ABSTRACT. The subject of this review is the role of soluble transferrin receptor (sTfR) to define iron status. After a description of iron metabolism, the biochemistry and regulation of soluble transferrin receptor are dealt with. Subsequently we discuss the physio-pathology of the molecule, its biochemical clinical role and then the analytical methodology used for its measurement. Finally, we mention some data concerning the reference intervals and the biological variation of sTfR.
Analisi nefelometrica di proteine del plasma: valutazione di un nuovo strumento

ABSTRACT. The aim of this study was to evaluate a new dedicated immunonephelometer for plasma proteins determination. We estimated the instrument's performance in the measurement of immunoglobulins (IgG, IgA, IgM), transferrin and C-reactive protein. We considered: system stability with or without repeated calibration; linearity; overall analytical imprecision; turbidity interference; inter-methods comparison by using Beckman Immage and Roche Modular as alternative systems. The results can be considered "state of the art" concerning overall imprecision (CV values between 3.7% and 15.6%), stability, linearity and sensibility to turbidity. Inter-methods comparison showed systematic differences, generally smaller than the critical differences except fot IgA, which showed, at the upper limit of the reference interval, an inter-methods systematic difference (-15.6%) correspondent to the critical difference's value (±15.5%).
