Vitamina D: risultati di uno studio osservazionale in età geriatrica

ABSTRACT
Vitamin D: an observational study in elderly. Vitamin D has gained a great interest over the last few years for two main reasons: 1) many epidemiological studies have shown a widespread deficit of vitamin D at every age; 2) many organs and tissues have vitamin D receptors. In the present study, we evaluated concentrations of vitamin D in 225 elderly subjects admitted to a rehabilitation unit in the North of Italy; we also assessed nutritional status (including body composition measured by bioelectrical impedance analysis) and inflammatory status (fibrinogen and C-reactive protein). Analysis was carried out by age, gender, timing of blood sample (summer vs. winter), nutritional and inflammatory status. We found a high rate (77.3%) of vitamin D deficit (in many cases of severe deficit) in our elderly population; a slight inverse correlation between fibrinogen and vitamin D was also found.

Impiego di 2-fluoro-2-deossi-d-glucosio marcato nell’imaging nucleare: effetti su una possibile contaminazione batterica.

ABSTRACT
Use of 2-fluoro-2-deossi-d-glucose in nuclear medicine imaging: effects on possible bacterial contamination. The 2-[18F]fluoro-2-deossi-d-glucose [(18F)-FDG)] is the most frequently used radionuclide for positron emission thomography (PET). The reason for its oncologic use lays in the high glucidil activity that characterize neoplastic cells. To allow its use in the clinical setting, it is necessary that (18F)-FDG preparation fulfills the following requirements: chemical and radiochemical purity, sterility, and apirogenicity. In our study, we observed what it would happens if the (18F)-FDG, handled without observing all sterility procedures, suffered a contamination from bacteria, typical of hospital environment, such as Staphylococcus Aureus. 300 μL of the solution containing an original stock of Staphylococcus Aureus ATCC6538 was injected into (18F)-FDG samples, with increasing radioactivity (from 185 MBq to 1850 MBq) produced by synthesis modules (GE Tracerlab and IBA modules). The bacterical growth and the antibiotic assay, both of colonies from the original stock and of those created in radioactivity, were evaluated. The sensitivity to amoxicillin/clavulanic acid, ampicillin, gentamicin, and ceftazidime was estimated. We observed a decrease in the growth of bacterical colonies under radioactivity. Moreover, a reduction of sensitivity to tested antibiotics was observed. Our study shows that the ionisating radiations may reduce the Staphylococcus Aureus growth and cause a reduction in the sensitivity to antibiotics. This decrease seems to be proportional to the radioactivity level. This study emphasize the importance of the right use of sterility procedures in production, preparation of doses and supplying of (18F)-FDG.

Valutazione dell’analizzatore Menarini SediMAX per l’esame automatizzato del sedimento urinario

ABSTRACT
Evaluation of Menarini SediMAX analyzer for automated urine microscopy analysis. The aim of this study was to evaluate the analytical performance of SediMAX (Menarini Diagnostics), a new analytical tool for automatic analysis of urinary sediment. On 422 routine samples, we correlated the microscopic examination with Sedimax morphological outputs. In an early experimental phase, the manufacturer’s ranges were adopted obtaining unsatisfactory results. To improve the degree of correlation, we modified the reference ranges. An overall concordance rate of 87.4% for red blood cells, 81% for leucocytes, 74.2% for bacteria, and 84.1% for squamous epithelial cells was finally obtained. The rate of false positive results ranged from 2.1% (red cells) to 17.3% (bacteria), and the rate of false negative results ranged from 3.7% (leucocytes) to 9.6% (bacteria). In conclusion, our results seem to suggest that the performance of SediMAX instrument should be improved before to recommend its use for replacement of urine microscopy examination.

ABSTRACT
Evaluation of Sysmex uf-1000i analyser for automated urine microscopic analysis. The evaluation of corpuscles in urine is used to investigate disorders of the kidneys and urinary tract. The objective our study was to evaluate the analytical performance of a new urinary citofluorimeter, the Sismex UF-1000i. The study was carried out by collecting 15 samples of native urine with concentrations of the analytes covering a wide measurement range. The imprecision was evaluated by processing all samples five times. Results were evaluated with reference to the mean of two readings with the microscope set at 400 X using the Fuchs-Rosenthal chamber count according to the European Urinalysis Guidelines. Results demonstrated a good performance for all the considered parameters, especially in the range of cut-off and decision making values.
